Surgery Consent

You must already have an appointment to complete this consent form. One consent form is required per pet.

If your pet does not have a valid rabies vaccine, it is mandatory at the time of surgery. If your pet does have a valid rabies vaccine, please bring a signed rabies certificate (tags will not be accepted) with you to drop off in the morning. If you do not have your rabies certificate any longer, please call the veterinarian’s office where the vaccine was given and have them fax it to (517) 908-0758.
